Eligibility: Patients of the hospital 15yrs. and under.
Will see older patients if affiliated with the hospital
as a patient.
Fees: Medicaid. No payment plans.
Services: Emergency care, sedation, pediatric general dentistry.
No oral surgery or braces.
Regular Waiting Period: Approximately 6 months for non-emergency

Walk-ins should come in by 5:00 – 6:00 a.m. for emergency care only. All
others must schedule an appointment.
Eligibility: Ages 3 and older; people with disabilities and medically compromised
treated.
Fees: Medicaid (all ages), insurance, cash
NOTE: Patients with no insurance must bring a minimum of $125.
Services: Oral surgery and emergency extractions. |

Eligibility: Open; appointment is needed. Must have a picture I.D. Must have
a referral slip from general DDS for oral surgery.
Fees: $60 at first visit for exam, x-ray, & consultation, Medicaid and reduced
fees
Services: Oral surgery and general dentistry. Emergency care only if a patient
of record. Sedation available.
Waiting Period: Approximately 6 –8 weeks. |

Fees: Medicaid (straight-no adult waiver), insurance, reduced fees for uninsured
Services: general dentistry, root canals and crowns. No sedation. Preventative
services done 1 day/week.
